Create an issue
Use this guide to log a new issue in the Issues register.
Before you start
You need Editor or Owner access to create issues. Viewers can read the register but cannot add or modify issues.
Make sure your organisation has initialised the Issues lists in Settings → Manage lists before creating issues.
Steps
1. Open the Issues register
In the project sidebar, click Issues under the Delivery section.
2. Click New issue
Click the + New issue button in the top-right corner of the Issues register page.
3. Complete the Issue summary section
- Title (required) — A short, descriptive title for the issue. Maximum 120 characters.
- Event description (required) — Describe what has already occurred. Be specific about what happened, when, and where. Maximum 1,000 characters.
- Priority — Select the priority level from your organisation's configured list (Low, Medium, High, Critical).
- Identified on — The date the issue was first identified. Defaults to today.
4. Complete the Cause and effect section (optional)
This section helps you document the root cause and downstream impact of the issue.
- Cause description — Describe the factor or event that caused the issue.
- Cause category — Select a category from your organisation's configured list.
- Effect description — Describe how the issue affects project objectives.
- Impact category — Select a category (e.g. Cost, Schedule, Scope, Quality).
5. Complete the Ownership and response section
- Owner — Assign the issue to a project member. Defaults to you.
- Due date — Set a target date for resolving the issue.
- Response description — Describe the action or approach being used to address the issue.
6. Click Create issue
Once you are satisfied with the information, click Create issue to save. You will be taken to the issue detail page.
What happens next
After creating an issue:
- The issue is assigned a reference code (e.g.
ISS-001) automatically. - The status is set to Open.
- A status history entry is created recording the creation.
You can edit any field later from the issue detail page by clicking Edit.
