Issues
What is an issue?
An issue is something that has already occurred — a problem, event, or situation that is actively affecting the project and needs ownership, a response plan, and eventual closure.
Issues differ from risks. A risk is something that might happen in the future. Once a risk event occurs, it becomes an issue.
The Issues module
The Issues module gives you a structured register for logging, tracking, and resolving project issues. Each issue has:
- A unique reference code (e.g.
ISS-001) - A title and event description
- Optional cause and effect analysis
- Priority, owner, and due date
- A response description explaining how the issue is being addressed
- A full status history for governance and audit
Status workflow
Issues move through three statuses:
| Status | Meaning |
|---|---|
| Open | The issue is active and being tracked |
| Paused | Work on the issue is temporarily suspended |
| Closed | The issue has been resolved or is no longer relevant |
You can move issues between statuses at any time:
- Open → Paused or Open → Closed
- Paused → Open (reopen) or Paused → Closed
- Closed → Open (reopen)
A closure reason is required to close an issue. A reason is required to reopen an issue. This ensures there is always a clear audit trail.
Issues are never deleted
Issues are a permanent governance record. Once logged, an issue cannot be deleted — not by editors, owners, or administrators. Closed issues remain in the register and can be filtered out of the active view.
This design is intentional. Project audits, post-mortems, and compliance reviews depend on a complete, unaltered history of what occurred on a project.

Issue register
The Issue Register lists all issues for a project. You can:
- Filter by status (Open, Paused, Closed)
- Filter to overdue issues only
- See priority, owner, due date, and impact category at a glance
Overdue issues
An issue is marked as overdue when its due date has passed and it is not yet closed. Overdue issues are highlighted in the register and shown in the sidebar health indicator.
